How To Become A

CAAP Member

Applying for Membership

If you are interested in joining CAAP, please contact us at 706-208-1990.  Our Office Manager will direct you to our Provider Relations Manager, Jessica Gay, who will be happy to assist you.  

CAAP is an Independent Physician Association (IPA) that is governed by its physician members.  Our Board of Directors is composed of Primary Care Physicians and Specialists, which ensures that all our physician members are equally represented.

CAAP also owns a Clinically Integrated Network, which is also governed by its physician members.  In order to join our Clinically Integrated Network, you must be a CAAP member. 

Applying for Membership

Thank you for your interest in becoming a CAAP IPA Member. Potential members will go through CAAP credentialing process.


To begin the process providers must submit:


  • GA Uniform Allied Healthcare Practitioner Application, Part I and II with current signatures. 
  • Current Certificate of Malpractice Information
  • W-9
  • CV


Additional forms will be sent from CAAP Credentialing to include:


  • CAAP Physician Participation Agreement (Signature page #15 & the last page)
  • CAAP Approval Form
  • CAAP Non-Disclosure Agreement
  • CAAP Membership Application
